Yes, they do have names. No, I'd prefer to avoid naming any from my
personal knowledge, as situations change and I would prefer not to
accidentally make a false accusation. With that in mind...
K3ZO, who now manages the W3 Incoming buro, posted to this list back on July
15th listing 3rd District (call) stations who decline to use the buro for
incoming cards. I recognize well known QSL Manager W3HC on that list, and
yes, that is one of the calls I had in mind. You can find the updated list
in the center of

there are probably other QSL managers there as well.
I also received a PM from another QSL manager, an old friend I haven't heard
from for awhile, who also discussed this. This individual WILL answer buro
cards from US amateurs for his stations... eventually. They tend to go to
the bottom of the pile.
Personally... look, if you really want a particular QSL card from a
particular station, doing it their (or their manager's) way is the cost of
doing business. (Are the policies of some outrageous? Sure, but luckily,
that's a very tiny minority.) That's just the way it is.

Do these masked men and women have names? Is there a way to find out in
advance?
To my knowledge, I never had a stateside QSL manager refuse a QSL card,
including back before we had stuff like QRZ.Com.

Don't it always seem to go that you don't know what you've got...till it's
gone.
