[Dx-qsl] Excel file for inputing log manually?

I remember someone had made a file in Excel to quickly enter the 
callsign date and frequency to export it for LOTW.. Has anyone a copy 
they could send to me as an attachment or a link to it??
I have an urge to put some of my paper logs on LOTW.. Many tnx in 
advance and Happy Holidays!
