Graham,
Stateside managers/direct respond 1-3 months unless they post that cards
just went to printer or it was a major DXpedition. Stateside follow up at 6
months with email or another card.
Overseas respond 6-12 months with German QSL managers being the fastest at
~1-3 months. The French second at 2-4 months. Email at 12 months to verify I
was "in the log" and asking if I should send another card. At 24 months I try
one more time with email/another card and write of the contact at 36 months.
All cards I send have an SAE with $3.00 US for overseas, SASE and a $1.00
donation for stateside. On a few occasions I forgot to put a stamp on my return
envelope, usually got a note with the confirmation QSL card from the manager
that stated he took it out of the funds that other Hams donated. I now pay
back the kindness with every stateside card.
I have been licensed ~20 years and am a "casual" DX chaser working only the
countries I need. I have 260 confirmed and WAS on 3 bands. Don't chase
certificates and the cards on my wall are my proof that I did indeed work them.
During that time I have "written off" about overseas 10 contacts. Zero to
stateside managers and 15-20 direct stateside.
I try on to get to frustrated after all it's a hobby and it's fun to watch
your country or stateside totals grow.

P.S.
Never, ever "kick the cat" it may take years but it'll get even......

Out of interest, and ignoring MAJOR Dxpedits, how long do you all wait
before crying in your beer or sending dupes or kicking the dog or
whatever it is you do, because the hoped for cards havn't materialised ?
I ask because I always try my hardest to respond to any requests within
48 hours - even if I am 'playing radio' [or kicking my dog because my
confirmations havn't come]
Am I an exception [or mad ?]
Graham
/think - No QSL Managers *are* mad - we have to be.
