Don't think GUANTANAMO BAY, CUBA belongs in the last line. The Zip code
tells the APO/FPO mail system what base the mail will go to. By adding
GUANTANAMO BAY, CUBA it may have caused it to be actually sent to the
Cuban mail system or confused the postal workers to return it to you as
undeliverable as addressed.
