Is there a list of the proper way to enter the Country names?

I see some address will say
Soviet Federation
or 
CIS
or
Azores 
St. Helena, South Atlantic Ocean.

Is there a list the Post Office uses as the official way to enter the name of countries?
I found a PDF on the US Postal Site but it's a bit ponderous.
And once I got out of it...I couldn't get back in. hihi
