[Dx-qsl] Not in Log

There are very few surprises left.  With a major DXpedition I usually
have many band and mode contacts.  I am no longer surprised to find
that either at QSLing time or in the on-line-logs that one of my
contacts has been 'lost'. I am very careful to make sure the DX
station has my correct call and when there is any doubt I will try
and work them again.
What is happening?  Are some of the operators on these DXpeditions
poor operators; is it because of mistakes made with computer logging;
is it my Canadian accent or cw signal?
When the 'lost' contact is for a needed band it can be discouraging.
Unfortunately the only way is to 'dup' the DXpedition on the needed
band(s).  This duplicating has the effect of taking away someone else's
contact and adding another signal to a crowded portion of the band.
I have contacted the QSL managers in an attempt to locate the 'lost'
contact and in most cases I am still 'not in log'.  So what's the
best answer?
For example the TX9 logs just became on-line.  So sure enough one of
my solid contacts is missing (thank goodness it wasn't a 160M contact
that I never made), and this is amazing, I am shown as having a RTTY
contact that I never made.  Someone else will be getting a not-in-log
for a RTTY contact that was theirs and not mine.
