I received one FR5ZU card via VE2NW.  It was a multi-use card, meaning FR5ZU 
/___ for various FR operations.  VE2NW filled in the appropriate bubble (/G, 
/J, etc). This was a few years ago, so you might e-mail Zareh first.  Good luck
