[Dx-qsl] Fees for LOTW now set

The basic fee for a QSL record issued for award credit will be $ 0.25 
(USD 0.25). Quantity discounts will apply depending on number of credits 
purchased.

Credits can be purchased according to the following table. Credits may 
be purchased at any time and in any price category. You may purchase any 
number of credits at the same cost, up to the next break point.

   1. Credits may be applied toward ARRL awards supported by Logbook of
      the World. Arrangements for non-ARRL awards are pending.

   2. Logbook fees are non-refundable and non-transferable.

   3. Payment may be made by credit card, check (US funds drawn on a US
      bank), international money order or US currency (at your own risk).

   4. Written orders may be addressed to Logbook Administration, ARRL,
      225 Main Street, Newington, CT 06111. Please include payment.

   5. Orders may be placed via fax sent to the attention of Logbook
      Administration (1-860-594-0259), telephone (1-860-594-0206), or on
      line via our secure order site.
