[Dx-qsl] 4M5A from CQWPX PHONE 1994 info needed

I think this call is being confused with 4M4A, NOT 4M5A. Mike K3UOC (4M4A) is NOT
the mngr. for 4M5A.

Maybe you can help us out here Mike, who has the log's for 4M5A, do you know? Thank
