B & W 551 "Transfer" or "Bypass" Coax Switch
This switch is designed to switch an auxiliary piece of
equipment in and out (For example, bypass a linear
amplifier, transverter, preamp, etc.).  It has four SO-239
connectors coming out the sides. 
I found another one.  It is in good shape for $40.

B & W 375 "Protax" coax switch
This switch will switch up to 6 antennas, and DOES
ground the unused antennas.  It has the coax coming
out the back in one large bundle.  This one works 100%
electrically, and looks very close to new.  $45
