Both!
The trick is to expose yourself to all sorts of code experiences. Contests
are good for this, as are W1AW practice sessions, computer software, ragchews,
etc.
What I have found helps the most is to listen to code that is so fast you can
only get 30 to 60% of what is being sent, no matter how hard you bust your
hump. (For practice only!) If you are in the low 20s, 30-35 or even 40 per might
be in that range. After 15-30 minutes of that, 25 wpm will sound slow!
