[CVRC] On-Line Field Day Application Error

OOPS!

There was an error in the on-line Field Day Application and BBQ Sign-Up 
form at http://www.cvrc.net/fd/fd2003-app.html

Naturally, the error-checking mechanism in the form didn't work and instead 
of telling you (or me) there was an error, it happily tossed the form into 
the trash and thanked you for submitting it!  ARRGGGHHHH!!!

I have fixed the form (REALLY!) but I need to ask everyone who filled it 
out to do it again.  Someone (probably Melissa) will send everyone who 
submits the form an e-mail acknowledging its receipt.  In the meantime, I 
need to apologize to everyone for the confusion and inconvenience.

Ain't technology grand?!
