[CTSARA] Hospital 440 Fully Implemented

Well, second time is the charm.  We corrected the mapping on the Arcom-Icom 
cable and installed it this morning.   It works, it works.  The hospital 440 now 
seems to be fully operational with proper sound levels, ID, courtesy tone, etc.

It does appear that the hospital 440 is monitoring 2 meters and is 
rebroadcasting anything being received on 2 meters.  I will try to turn that off 
later, but this will be my first attempt at doing something like that.  If 
anyone knows how to turn off the monitoring, please turn it off.
