[Collins] Collins transformer

I have a Freed transformer with a Collins part number of 674-0673-00.
The Freed number is 36747.
The dimensions of the transformer is 2x2.5x2.9 inches.
The transformer weighs 25.4 oz.
The transformer is a sealed transformer with 4 studs for mounting.
The transformer has 12 terminals with 4 windings.
The windings are terminal 1-2, 4-4, 5-6-7-8, and 9-10-11-12.
It appears that the equipment that contained this transformer was 
scrapped due to a quality control problem during installation of the 
transformer.
Terminal 12 was never soldered.
Does anyone recognize this transformer?
Google has been no help.
If it helps, the transformer still has enough wire still attached to 
the terminals to determine the colors of the wires.
Any help appreciated.

I will probably scrap the transformer if I cannot find data on it.
