You need an Amphenol 97-3106A-12S-3P. $10.39 in my Mouser catalog
(www.mouser.com). You also want a cable clamp 97-3057-1004 with bushing
or 97-3057-1004-1 or 97-3057-4 at $6.89 to 7.56.

These should also be available surplus at places like Fair Electronics
using the prefix MS instead of 97.

Probably there are dozens in connector boxes around the country too.
