Hi Larry,
My 75A4 is just as you described! I would really appreciate it if you could
copy the insert and send it my way. Be glad to pay expenses.
I figured it had to be some sort of military convsersion, as it is
professionally done.
The bandswitch has a plate with the new band markings engraved on it mounted
over the original markings. The linear (slide rule) dial is nicely done
with new freqs also.
I can take a digital photo of it if you are interested.

I picked up an original 75A-4 manual at a recent hamfest and it had the
following insert.
75A-4 Modification for BUSHIPS. 520-5468-00, 1 June 1956.
Takes the 15 meter band and makes it cover 4.2 - 5.2 mc, the 11 meter band
and makes it cover 13.0 - 14.0 Mc and the low 10 meter band covers 5.2 - 6.2
Mc. I don't recall seeing anything about this previously. Is this
something unknown or something that may have been covered several years back
before i associated with this list? I've never heard that this was done for
the navy. It's all typewritten like it might have been done at the factory
cause it's certainly not in the style of some ham or off the wll mod.
