I have one I found that appears unused. Millen 10039. Looks like the card will come out & can be scanned. Take it into a photo editor to refresh the background color (mine looks brownish, probably not the original color) or to make a finished version with your band markings.

I think I have a couple of those vernier pointer dials, of course the scales are no good. Wonder if using modern printing/copying the scale can be replaced.
