I've used ladder line, and old TV twinlead for years. My current ant. is about 110 feet of wire fed in the centre with W7FG ladder line and a KW-EZEE match tuner. Works great on 80 and 40 and with patience it will satisfactorily load on higher bands as well. I also use an old Johnson Matchbox and it also works great.
Don't sweat how to bring the ladder line into the shack, My line runs under a windowsill, takes a couple of 90 degree turns and in places, narrows, but it loads fine and I have no RF in the shack issues.
I started using ladder line when 1st licensed in 1972 as it was cheaper than coax and I was in high school and had zero money.
I agree with the other comments: don't use a tuner with a balun. I see old KW-EZEE matches at Hamfests for around $25 and Johnson Matchboxes for around $50 so use one of them or other such open wire tuners.

Hoping to get my open wire W7FG doublet (125') up this weekend (trueladderline.com).
A 3' length of coax runs from the AT2KD through the wall into a weatherproof junction box (arrestor and balun w/open wire terminals).
Last week I picked up a Kilowatt Matchbox as part of a lot which included a HW-100 and PS ("Grandpa talked to Australia on it").
Should I consider using the Matchbox instead?
If I did use the Matchbox, would it be necessary to bring the ladder line into the shack?
Trying to catch up on everything I didn't learn while away from the hobby for 50 years.......
