Ray,
I'm working from the SB-301 schematic. There's negative feedback from
the output transformer secondary to the 1st audio cathode. (A great
feature) That means, that withing reasonable limits, the amp will
provide the same output voltage regardless of load. Think about it. The
16 ohm speaker in going to draw only 1/4 of the power delivered to the 8
ohm unit. That's -6dB. Plus there may be differences in speaker
efficiency.
I can't say much about you second problem. However, I wasted a lot of
time recently with similar symptoms in an old Motorola car radio. There
was internal leakage in the duo-diode-triode, (10 megohms from diode
plate to triode grid.) and some of the signal was completely bypassing
the volume control. Change the tube!

At the lowest setting, the tube should not have enough, if any, signal to amplify, but somehow some signal is getting past the volume control and the tube is amplifying it. At minimum, incoming audio should be going to ground through the pot.
