The original was probably an Amphenol type - here's a used one

What you want is
2 pole, 3 wire, grounding, 15 Amp, 125 Volt, Flanged Inlet, Straight Blade

Some currently available ones are:
Hubbell HBL-5240 is listed as a "miniature version" for use with molded
cord receptacles. It might work. Dimensions are here - Looks like it may
fit OK.

Leviton 5239 - maybe too big for the existing hole.
