[Boatanchors] Hickok 1704 Tube Tester Adapter

I have a TV-7 tube tester and would like to buy or build a Hickok 1704 Tube Tester Adapter.  I doubt I can find a real one to buy, but maybe someone on here has one and could ring out the connections for me so I could make one.  Or maybe someone has the information at hand.  Any info or help would be appreciated.  (This adapter allows you to test 9 pin tubes such as 6JB6 and 6Gj5 tubes which are TV sweep tubes.)
