Radio Shack 274-312B for the 1/4" jack, Chinese made, but not all that bad
of quality in the "B" version, is Tip ring sleeve stereo jack.
274-1546 for the Tip Ring Sleeve 1/4" plug, metal housing and spring strain
relief.  Not too bad either.

If all else fails, there is always Radio Shack.  Virtually all plugs and
jacks are made in China today.  However, I have had pretty good luck with
those from Radio Shack.
