Yes, you have to take the Amateur Extra Class examination to upgrade.  However, the test is not that hard!

Go to the ARRL website and you can download the entire exam pool for nothing.  Also, go to http://www.qrz.com and you can take practice exams for free.  I am trying to remember, but I believe that after you take like 20 examinations you have gotten all of the questions.  However, it may be more!

Slightly off topic. I currently have an old advanced license. Do I need to take the extra test to upgrade? If so, are the test pool questions available without having to buy the ARRL study guide? There is no reason for me not to upgrade, except for finding the time.
