[Boatanchors] National "Paper tags"

On the bottom of my receiver (National HFS) there are two paper tags that I 
need to duplicate.

One, the "Form 227" is intact and readable but the other one "Standard form 
warranty" is missing parts and is unreadable.

 This is the one that says "Standard form warranty" and "Adopted by the 
Radio Manufactures Association, Inc."

Does anyone have a good one that they could read and send me the text?
