Excellent explanation and research Glen!  With regards to the power 
supplies.  Millen never made one to supply power for the 90881 amplifier 
as strange as that might seem.  They had a power supply 90281 called the 
"High Voltage Supply", but alas it only provided the 700VDC (and 
filament AC) for the 90800 or 90801 transmitters.  Millen also had a low 
voltage supply designed for the B+ and bias voltages of the low level 
stages.
