A few years ago, I was put on signature over a $40 item I'm convinced 
they mis-delivered.  This happened in a week when I had several hundred 
dollars of audio gear delivered, so I really picked the wrong one for fraud.

After about two years of having to be here to sign for UPS deliveries 
and unhelpful responses locally and from customer noservice, I sent a 
letter to the office of the chairman detailing the incident and why I 
thought the item was mis-delivered rather than stolen.

A couple of weeks later, the local manager was on the phone wanting to 
know how he could fix it.  The chairman's people had sent him an email 
wanting to know what was going on.

Clark Howard, a consumer advocate on the radio in Atlanta, recommends 
contacting the president or chairman with complaints which aren't 
settled at lower levels.

It worked for me.
