Not all of the older transmitters can have
neutralization added without some major surgery. 
Also, taking the "lazy man's" approach, why modify the
transmitter when it will work fine with the earlier
versions of the tubes.  The earlier versions are still
available (in fact are still being manufactured "off
shore").

As I have been saying all along, if the transmitter
will neutralize and hold the neutralization then the
6146B can be used without any problem.  It is the fact
that this is not possible in many of the older
transmitters, some of which have the capability to
neutralize and some don't.

If the owner wants to try the 6146B then so be it. 
There is a good chance that the tube will work. 
However, there is an even greater chance that the
transmitter will not work properly with the 6146B in
place of the earlier tube.  

Again, the 6146B is a different tube from the earlier
versions.  It is rated at 33% more power, it has
different voltage requirements, and is of a different
internal construction.  RCA chose to call the tube a
6146B when it would have been better to give it a
completely different nomenclature.  But, RCA did what
they did.  Unfortunately, that has caused problems for
over 41 years.

On radios with no neutralization that is different.
The DX 100 can be modified to add neutralization, the
Valiants have it.  Don't know about any others but it
shouldn't be a great problem to add..
