My experiences are similar to John's.

During the last two years or so I have converted several dozen Drake
transmitters to 6146 finals.  I use new 6146Ws.  Never had a problem.
The neutralization had to be tweaked along with the bias, but once that
was done everything worked as expected.

I still have the orginal T4X that was converted about four years ago. 
Still works.  Still going strong.  I also have a T4XC that was converted
about a year ago.  Works well and gets a regular weekly workout.
