If the house is wood frame, I have encountered cases of a staple driven
through the HOT (black) conductor. The intelligent , cost conscious
contractors then cut the ground loose at the main panel rather than go
through the expense and effort of replacing the run of Romex.
When I attempted to connect the earth ground, that mistake almost cost
me pair of Kenwood "Twins"! The arc certainly did engrave a small
portion of chassis. I was lucky; I did not form a path (the old "keep
one hand behind your back" rule) between the earth ground wire and the
chassis ground connection bolt.
