My normal usage:
    ham radio
    amateur radio (the hobby in general)
    Amateur Radio (the hoby as a single  entity)    
    Amateur Radio Service (formal FCC title)
 
Other than the last, none of these are "officially correct": the  Associated 
Press Stylebook and the Chicago Manual of Style  have nothing to say on the 
subject.
 
I have also found a number of people who use "HAM radio," as though they  
were dealing with an acronym.
 
The nearest thing to a standard that I would recommend is to stay  consistent 
with usage in the ARRL publications and web page.
