Hi Rod,

Here's a sample that you are free to use:

It is our membership application.  On page 2 there is a release of
liability stating:

"The undersigned assumes the unavoidable risks inherent in all Martin
County, FL ARES/RACES, Inc. related functions, including but not
limited to bodily injury and physical harm to a Martin County, FL
ARES/RACES, Inc. member, the undersigned's family member or
spectator.  In consideration, therefore, for the privilege of
attending any Martin County, FL ARES/RACES, Inc. function the
undersigned does agree to hold harmless and indemnify Martin County,
FL ARES/RACES, Inc., its Officers, Directors and other Martin County,
FL ARES/RACES, Inc. members, and further release them from any
liability or responsibility for accident, damage, injury, or illness
to the undersigned or to any family member or spectator accompanying
the undersigned at any Martin County, FL ARES/RACES, Inc. function. I
will fully comply with the Constitution and By-Laws of Martin County
FL ARES/RACES, Inc."

My understanding is that such releases really don't mean much (of
course that depends on the laws in your location) because State and
Federal legislation may specify who is liable or possiblely immune.
