The most commonly accepted forms among our customers (the emergency
management agencies and NGOs) is the ICS form 213 as a message form.

The form 213 is not exactly 100% identical everywhere, but the
variations are minor enough that anyone trained on the form from one
source should have no trouble with anyone else's version.  Here are some
typical examples:

The log sheet was not among the initial ICS forms.  Most agencies used
either their own regular duty log form (generally copied from the Army
Staff Duty Journal log DA form 1594

modified to better suit a communications log.  Some SAR organizations
call the form # 133 other agencies call it form # 309.
