[Ares-races] repeater use agreement

Our ARES group targeted our request to use repeaters direct to the 
repeater trustees.  Only one of the four repeaters granting approval was 
owned by a club, which required a vote of the membership.

Repeater Use Agreement

By my signature below, I grant permission for the North West Harris 
County Amateur Radio Emergency Service to use the ___________ repeater 
(check all that apply):

    __ during communication emergencies

    __ to conduct a weekly training net at 8 PM Sunday evening

    __ to conduct training drills and exercises at other times 
throughout the year

My permission may be withdrawn by me at any time, without prior written

__  This repeater is not to be used for ARES purposes.
