Actually, if I am looking at the correct handbook, it is chapter XI.

Anyway, I found Mark's exact circuit in the RCA Radiotron Designers Handbook. fig 33.14 
on page 1221.

A 6SL7 connected in cascode controlling the pass tube(s).
