Neil,
When did they change the Australian-NZ-Pacific Islands plug so that the top end of the pin is insulated.
I bought an adapter about six years ago from radio shack and the pins are not insulated?

Good points, except that positioning Earth (what you call ground)
to the right would be safest, since the Active pin is on the right
as you look at the pins end on.
I don't know why the sockets are oriented as they are, except that it
was apparently decided by 1937 according to this, which also shows
pictures of the Australian-NZ-Pacific Islands plug and socket.
