Dave could not be more correct.  The fact is, ARINC runs most of the 
CONUS and a few OCONUS HF ground stations for the FAA and air carriers.  
The USAF and Navy also operate long-distance HF and have a plethora of 
channels.  The Navy operates "Giant Killer" off the Virginia coast on HF 
and UHF.  The USAF also still operates their strategic HF nets.  Throw 
in a number of discreet channels for the USCG, too.  A number of pubs 
cover the military and civil HF frequencies.  There must be at least 150 
active channels from 2-26.5 MHz.  Per ICAO and NATO, etc. most, if not 
all, are USB.  Now, all said, some air carriers do have satellite 
service available.
