Have you tried a new 12A6?

I agree with both John and Robert: 50mV of hash on top of 250 VDC is very 
low, and you should not be hearing it at all in any normally operating radio.

Somethine ELSE is wrong.

Have you replaced any/all of the "flower pot" can-caps? If you haven't, at 
least one of those is shorted, leaking or open....or soon will be.
