In my experience with helicopters and later quality control in  
industry, most individual disasters and big screw-ups happen because  
of a number of factors.  Maybe not too many, or maybe a long string of  
them.

So I suspect that some or a number of the factors above were to be  
responsible for the Russian aviation safety record, "incident by  
incident".  YouTube has quite a number of Soviet/Russian rocket/ 
missile disasters (and of course a number of US ones, too.)

A moment of silence, please, for the frozen O-ring.
