Those potted modules were discrete component on a small PCB.
Burr-Brown tended to be rectangular; Analog Devices and Philbrick tended
to be square.
They needed few pins:
+15
GND (sometimes)
-15
+ Input
- Input
Output
Trim (sometimes)
-John
