It happened to me.  My choke was wired in the center tap of the plate XFMR
and then to ground.  The insulator on the choke shorted out and it grounded
the center tap of the plate XFMR, effectively bypassing the choke.  Voltage
nearly doubles.

Sounds like a shorted filter choke.
