After listening to a 'Swap Net' on SSB last night, it's my understanding 
that the paper work you are seeking is a rare-item commodity, indeed..
however, said item  -can- be had.

Contact K5MDM, W5JMD or WQ5J and see if a copy isn't available somewhere.
