I designed this with Windows Excel and there it works great.  However when I opened it in the default Windows spreadsheet, it was mal-formed.  So download it first and open in Excel or Libre Office and only print pages 1-2.  

I'll try to fix this soon so it will open correctly in other spreadsheets.
