[ADXA] DXCC application - paper card LA5CI/P Jan Mayen

Thanks, Lanny - I use DXLab.  This is a card for LA5CI/p and  it is not
even entered into DX Keeper (logging module for DX Lab).  It was also under
my previous call.  I was just going into the online card submission process
to get the cards ready for checking.
