[ADXA] Thanks to Scott for this idea. I took it and ran with it.

I had created a DXP spreadsheet some time back and was about to post it
with all the recent DXP activity.  But Scott beat me to it and in a
more elegant way.  So I took my attempt and merged it with Scotts grid and
came up with this one.

The opening TAB is a master creation sheet.  Just fill in the information
in the PINK area and follow the "sort" instructions and you can have up to
12 DXPs at your fingertips.  This one is designed to be printed on both
sides (duplex) and then just fill in whatever DXPs you want.  Fold it in
half longways and you have 1-3 down.  Flip it and you have 4-6 down.  Open
it and fold the other way and you have 9 and 12.

It's currently populated with those DXPs coming up from the ADXA website
for starters.  If you want to just print several blank forms and fill them
out as you need, the MANUAL tab at the bottom can be used to print as a
blank form for you to fill out with whatever information you want.
