[ADXA] ADXA Certificate of Affiliation

Attached is a "pdf" file showing the ADXA CERTICATE OF AFFILATION that 
Stewart found somewhere. I didn't know it existed until I received the file 
from Stewart. A good many years ago K5YY sent me copies of original minutes 
from which I composed a short history of the ADXA. I am going to ask 
Stewart to bring the certificate to our next meeting.
