[1000mp] How to reset Mark V to factory defaults?

How can I reset my Mark V to factory defaults?

My Mark V is acting very funny including no output on 20 meters.
The swr on that band is full scale on the meter.

There is no information how to reset that I could find in either my Mark 
V Operating Manual or the Overview booklet.  Is there some other manual?

I vaguely recall a hard and a soft reset but have long forgotten the 
details.
