I run the VFO A and VFO B audio from the back of my Mk V into channels 1 
and 2 of a 4 channel mixer.  I route the L and R outputs of the mixer 
to, among other places, a headphone amp.  I hear the audio from VFO A in 
my left ear and the Monitor output in my right ear (Mk V MONI button 
"ON").  Both levels are independent of front panel control settings but 
each one is controlled by the sliders on the mixer.  To hear both 
outputs in both ears I flip the "MONO" switch on the mixer which 
parallels the L & R outputs of the mixer.

I don't know anything about the Soundblaster but it seems to me you 
could accomplish the same with a couple of pots on the AF Out lines 
feeding a simple resistive combiner to get everything onto one channel.
