These things can drive you crazy but you might just be looking in the wrong
place.  Don't forget that no amount of filtering will stop rfi if it's as a
result of rectification at a bad joint.  I have had this problem with
telephone lines before, where wires have been lightly twisted together in
junction boxes and not properly clamped by the restraining screw.
Rectification occurs where the two wires meet.  You'd be amazed at how much
trouble that can cause.  Worth a look.
